Commonly in general clinics they explain osteotomy as a complex surgery. Anyway it's simpler than one thinks. 5 minutes is enough to carry out osteotomy.
If osteotomy is necessary it should be done although many think it's a big surgery.
The following cases usually need osteotomy: hooked nose deviated nose flat bridge.
The bone by the glabella is thick so the osteotomy is carried out just until that area.
