This patient is in her twenties and have bulging lowerlid. The older is the patient, nomally lower eyelid could be bulging. In that case, resection od skin and removal of fat is accompanied. But for patinets in ther twenties, usually just fat removal is done through transconjuctival approach, and it needs no skin incision. So scar cannot be noticed.
Patients, like this case, who have bulging lower eyelids looks like nervous or tired.
