This is an example of lower eyelid blepharoplasty (see arrow). If there is a protrusion of the lower eyelid there is an irritated/tired/gloomy/old look. The skin of young individuals have good elasticity , we can therefore approach this problem via a transconjunctival approach and remove orbital fat. But in the elderly we need to excise skin and approach the surgery through a external incision.
